Candidates for membership in the Minnesota Academy for Comprehensive Dentistry must undergo the process described in the by-laws for membership. The number of “active” members is limited to no more than 50 (fifty). When an opening exists, any member can nominate a candidate by completing the Nomination form below, having a completed biographical profile for the candidate completed and have a second member complete a seconding nomination. The three “electronic files” (1. Candidate Biographical Profile, 2. Nomination form and 3. Second form) can then be e-mailed to the Chairperson of the Membership Committee to start the Nomination process. Alternately, the files can all be printed and submitted to the Chairperson of the Membership Committee.
Either have the candidate complete the Biographical Profile or complete it for them. All information requested should be included for the application to be considered complete. The Nominator must complete the Nomination Form below and obtain a letter (file) seconding the nomination from another MACD Member. All three items must be received by the Chairperson of the Membership Committee for the nomination process to begin.
